Please complete Witness Slips for EVERY member of your household (we have confirmed with Springfield that there is no age limit) in OPPOSITION of mandatory COVID-19 Vaccinations in IL and in OPPOSITION of BOTH of these bills.

PLEASE PAY CLOSE ATTENTION TO INSTRUCTIONS BELOW WHEN FILLING OUT THESE WITNESS SLIPS AS SOME ARE PUTTING THE WRONG CHOICE IN SECTION III FOR YOUR POSITION. WE ARE OPPOSING ALL THREE OF THESE.

1. Under Section I, fill in your identification information. For “Firm”, fill out your organization if you are representing one or write "self" if you are representing yourself. 2. In Section III, select "OPPONENT”
3. In Section IV, select "Record of Appearance Only".
4. Agree to the ILGA Terms of Agreement.
5. Select the "Create Slip" button.
